Job Description

This is a full-time social work role in the CAPE (Critical Care, Anesthesia, Perioperative, and Home Ventilation) Program. The program promotes high-quality, coordinated care to improve the lives of children who require long-term ventilator assistance and their families. CAPE services are tailored to meet individual patient needs and include extensive home visits, outpatient clinics, and inpatient consultations.

This CAPE Clinical Social Worker will be responsible for:
  • Conducting psychosocial assessments and provide consultation and support to families
  • Managing cases and coordinating complex resources with community, governmental, school, healthcare, and other agencies
  • Collaborating with a multidisciplinary team across the continuum of care
To qualify, you must have:
  • Masters in Social Work, and state of MA LICSW
  • Several years of social work experience in a medical environment with chronic illness highly preferred
  • Ability to thrive in a fast-paced, dynamic environment, along with flexibility and excellent communication skills
  • Bilingual in English and Spanish is preferred
